URGENT MESSAGE FROM RELAY HEALTH - DRUG RECALL NOTICE

Bristol-Myers Squibb is initiating a voluntary Retail Level recall
that addresses a safety issue with the product due to some tablets,
over time, may not meet specification for Isopropanol, which is
required to maintain the active ingredient in a crystalline state and
is asking pharmacies to now remove listed LOT #S for the following
product:

* Coumadin 1mg tablets (NDC# 00056-0169-75)
Lot #s: 8F34006B, 8K44272A, 8K46168A, 9F44437A, 9K58012B

Pharmacies are advised to quarantine and discontinue distribution of these
lots. Further instructions will come directly from your distributor.
