URGENT MESSAGE FROM RELAY HEALTH - DRUG RECALL NOTICE

AvKare is initiating a voluntary Retail Level recall that addresses a
safety issue with the product due to a broken label issue and is
asking pharmacies to now remove listed LOT #S for the following
product:

* Sulfamethoxazole-TMP DS tablets (NDC# 42291-0515-50)
Lot #s: HJ13209

Pharmacies are advised to quarantine and discontinue distribution of these
lots. Further instructions will come directly from your distributor.
