URGENT MESSAGE FROM RELAY HEALTH - DRUG RECALL NOTICE

Major Pharmaceuticals is initiating a voluntary Retail Level recall
that addresses an issue with the product due to a potential GMP
violation and is asking pharmacies to now remove listed LOT #S for the
following product:

* Carbidopa/Levodopa ER 50-200mg (NDC# 00904-5750-61)
Lot #s: P-13510, P-13825

* Clonazepam 0.5mg tablets (NDC# 00904-5342-61)
Lot #s: P-13127, P-13219, P-13390, P-13526, P-13611, P-13696,
P-13759, P-13847, P-13929, P-13974, P-14070, P-14083,
P-14192, P-14229, P-14299

* Clonazepam 2mg tablets (NDC# 00904-5344-61)
Lot #s: P-13839, P-13973, P-14069, P-14171

* Loratadine 10mg tablets (NDC# 00904-5793-61)
Lot #s: P-13382

* Omeprazole DR 20mg capsules (NDC# 00904-5684-61)
Lot #s: P-13555, P-13689, P-13787, P-13830, P-14060, P-14062

Pharmacies are advised to quarantine and discontinue distribution of these
