Origin Codes on Transferred Prescriptions

On new prescriptions, the following Origin Codes are required by some
insurances.

1 - Written Prescription
2 - Telephone Prescription
3 - Electronic Prescription
4 - Fax Prescription

Particular insurances are also requiring Origin Codes on prescriptions
that have been transferred from another pharmacy. The insurance
companies want to receive the following information.

2 - Prescription transferred by Telephone
4 - Prescription transferred by Fax

Pharmacies should not receive transfer prescriptions written or
electronically. The insurances will also not accept a 0 for
transferred prescriptions. Origin Codes of 0 will only be accepted on
prescriptions that have an Original Fill Date before Origin Codes were
required.
