URGENT MESSAGE FROM RELAY HEALTH - DRUG RECALL NOTICE

Teva is initiating a voluntary Retail Level recall that addresses a
safety issue with the product due to possibility that some bottles may
not meet fill weight specifications and is asking pharmacies to now
remove listed LOT #S for the following product:

* Amoxicillin suspension 250mg/5mL 80mL bottle (NDC# 00093-4155-79)
Lot #s: 35410395A

* Amoxicillin suspension 250mg/5mL 100mL bottle (NDC# 00093-4155-73)
Lot #s: 35400826A, 35405154A, 35405163A, 35405389A, 35405394A,
35405403A, 35406134A, 35407009A, 35414041A

* Amoxicillin suspension 250mg/5mL 150mL bottle (NDC# 00093-4155-80)
Lot #s: 35405034A, 35406014A, 35407134A, 35408993A, 35414055A,
35414062A

Pharmacies are advised to quarantine and discontinue distribution of these
lots. Further instructions will come directly from your distributor.
