URGENT MESSAGE FROM RELAY HEALTH - DRUG RECALL NOTICE

Qualitest is initiating a voluntary Retail Level recall that addresses
a safety issue with the product as some of the tablets exceed the
weight requirements and is asking pharmacies to now remove listed
LOT #S for the following product:

* Phentermine 37.5mg tab 100s (NDC# 00603-5192-21)
Lot #s: T058E09A

Pharmacies are advised to quarantine and discontinue distribution of these
lots. Further instructions will come directly from your distributor.
