URGENT MESSAGE FROM RELAY HEALTH - DRUG RECALL NOTICE

Teva Pharmaceuticals is initiating a voluntary Retail Level recall
that addresses a safety issue with the product due to some tablets may
exceed weight requirements and is asking pharmacies to now remove
listed LOT #S for the following product:

* Clonazepam 0.5mg tablet 100s (NDC# 00093-0832-01)
Lot #s: 35200435A, 35200408A, 35200409A, 35200436A,
35200437A, 35200460A, and 35200462A

* Clonazepam 1mg tablet 500s (NDC# 00093-0833-05)
Lot #s: 35200220A, 35200202A, and 35200248A

* Clonazepam 1mg tablet 1000s (NDC# 00093-0833-10)
Lot #s: 35200203A

* Clonazepam 0.5mg tablet 500s (NDC# 00093-0832-05)
Lot #s: 35200438A

* Clonazepam 1mg tablet 100s (NDC# 00093-0833-01)
Lot #s: 35200199A, 35200200A, 35200201A, 35200218A,
35200219A, 35200278A

Pharmacies are advised to quarantine and discontinue distribution of these
lots. Further instructions will come directly from your distributor.
