URGENT MESSAGE FROM RELAY HEALTH - DRUG RECALL NOTICE

Teva Pharmaceuticals is initiating a voluntary Retail Level recall
that addresses a safety issue with the product due to some tablets may
exceed weight requirements and is asking pharmacies to now remove
listed LOT #S for the following product:

* Clonazepam 0.5mg tablet 100s (NDC# 00093-0832-01)
Lot #s: 35200435A

* Clonazepam 1mg tablet 500s (NDC# 00093-0833-05)
Lot #s: 35200220A

* Clonazepam 1mg tablet 1000s (NDC# 00093-0833-10)
Lot #s: 35200203A

Pharmacies are advised to quarantine and discontinue distribution of these
lots. Further instructions will come directly from your distributor.
